Case Management
Those activities provided to assist and support individuals in gaining access to needed medical, social, educational and other services essential to meeting basic human needs.
Program Objectives:
- To ensure assessment of need and advocacy with other systems to meet the needs of SED youth and families, including housing, employment/educational status, health, income, recreation, cultural characteristics, and spiritual needs.
- To enhance the development of resiliency and assets of youth and families, to facilitate school success and future empowerment.
- To provide coordination of services with Juvenile Court, JFS, Schools and other community referral sources
