Diagnostic Assessment
Mental Health assessment is a process of gathering information to assess client needs and functioning in order to determine appropriate service/treatment based on identification of the presenting problem, evaluation of mental status and formulation of a diagnostic impression. The outcome is to determine the need for care and recommend appropriate services and/or treatment.
Program Objectives:
- Diagnostic Assessments shall be scheduled within 7-10 working days of the initial contact.
- Agency Levels of Care, Ohio Scales and SED Protocols shall be utilized as a guide for referrals to appropriate service environments.
- The Agency will ensure that Master’s level counselors with specialized training in assessments are primary assessors.
- Assessments will be conducted upon referral to determine appropriateness for specific program enrollment, e.g., Sex Offender, Filial Play, Homebuilders, etc.
