(Homebuilders Model)
Family Solutions is a program designed to preserve families in their natural
home environment and to assist them in gaining insight into their family
systems and the problems associated with emotional/behavioral disorders, as
well as substance abuse. Home based services and community
collaboration/support services are the focus of treatment to prevent
unnecessary out of home placements.
Program Goal
- Of the 120 families served by
the program, 112 families will remain in their home.
- To ensure that families are
able to maintain stability utilizing skills acquired, natural supports and
community resources.
Program Objectives
- Staff/client ratios shall be
1:6-8 families (depending on the needs of the families)
- All families served by the
program will have a community safety plan.
- All families completing the
program will have a continuing care plan upon discharge.
Target Population
Youth and families at substantial risk of out of home placement due to severe
emotional/behavioral disturbance and/or Substance abuse problems. The program
may last from one month to six months, with an average of 4 to 6 hours per week
in the home and community.
Expected Outcomes
- Outcomes will be measured by
utilization of a risk assessment, Ohio Scales and the North Carolina
Assessment, as these tools will identify family interaction and safety
issues, which lead to disruption and/or out of home placement.
- Clinical Precision Plus will
measure Homebased Outcomes and Customer Satisfaction.
- Performance Target Outcomes
will be evaluated Quarterly by the MHRSB.
Hours of Operations
Home-based Services are available based on consumer need.
