Wraparound
The Wraparound Model is based on individualized, needs driven planning and
services. It is an unconditional commitment to create services on a “one kid at
a time” basis to support normalized and inclusive options for youth with
complex needs and their families.
Program Goals
- To individualize services for
high-risk youth and their families in order to stabilize the natural home
environment.
- To prevent unnecessary
out-of-home placements.
- Of the 20 youth served by the
program, 18 of the youth will not be placed in an out of County
residential placement and will improve overall functioning at home,
school, and community.
Program Objectives
- To train agency providers and
community members in the VanDenberg Model of Wraparound.
- To expand the program to all
three counties.
- To collaborate with community
resources, traditional and nontraditional, in order to ensure individual
consumer needs are met in the least restrictive environment possible.
- To train community members on
Wrap Team facilitation.
Target Population
- Youth and adolescents between
the ages of 8 and 17 and their families.
- Youth, adolescents and
families who experience significant impairment of ability to perform in
the family, in school, and in the community.
- Youth, adolescents and
families who are identified by the community as multi-need and high risk.
- Youth and adolescents who are
at substantial risk of being removed from their natural home environment.
- Youth, adolescents and
families who have not responded to traditional community services.
Expected Outcomes
Outcomes and Consumer satisfaction will be measured by Clinical Precision
Plus. Performance.
Hours of Operation
Wraparound services shall be available based on consumer need.
