TELEHEALTH
Telehealth enables you to see your provider without an in-person office visit. Telehealth is primarily done online with a computer, smartphone, or tablet connected to the internet, and can be done anywhere that is convenient for you. There are no apps to download, accounts to create, or passwords to manage. We don’t collect any of your personal data, and we use encrypted audio and video technology to provide HIPPA-compliant services in each Telehealth appointment. Even if you aren’t comfortable saying things out loud, you can use the chat feature to type messages to your provider.

If you would like to get a head start on the enrollment paperwork necessary for becoming a new client, you can download and print the form below. Tip: when printing this document two-sided, select “flip on the short edge” so that the pages will be easier to navigate.
